The transportation infrastructure of the Appalachia HIDTA includes Interstate highways I-24, I-26, I-40, I-64, I-65, I-75, I-77, I-79, and I-81; U.S. Highway 127; State Route 111; and Cincinnati, Louisville, and Nashville International Airports.

Cincinnati, Ohio; Louisville and Lexington, Kentucky; and Nashville, Tennessee, are cities with populations over 250,000. Chattanooga and Knoxville, Tennessee, are cities with populations between 100,000 and 249,000. Huntington and Charleston, West Virginia, and London, Kentucky, are cities with populations less than 99,999.

In 2001, the number of drug treatment admissions for marijuana was 3,484; cocaine 1,644; pharmaceuticals 945; amphetamines 454; and heroin 169.

In 2002, the number of drug treatment admissions for marijuana was 3,753; cocaine 2,201; pharmaceuticals 1,128; amphetamines 455; and heroin 220.

In 2003, the number of drug treatment admissions  for marijuana was 4,614; cocaine 3,114; pharmaceuticals 1,646; amphetamines 696; and heroin 290.

In 2004, the number of drug treatment admissions for marijuana was 3,175; cocaine 1,815; pharmaceuticals 1,353; amphetamines 532; and heroin 197.

In 2005, the number of drug treatment admissions for marijuana was 3,346; cocaine 1,929; pharmaceuticals 1,403; amphetamines 746; and heroin 251.
