Clicking the Forms tab on this site's navigation bar, you can access the PDF files that contain BOTH the instructions and the compensation form.

Submissions MUST include original signatures, and the last signature MUST be notarized as indicated. You must also supply supporting documentation. Please use the checklist to help ensure you have attached the necessary documents. Please note that in most cases the documentation you submit with the claim will not be returned to you. Therefore, be sure to make copies or have additional certified copies of documents that you submit with your claim.
