If you received an E-QIP investigation request number, please follow the instructions below. If you have not received an E-QIP number, please disregard this process.

Upon completion of your SF-86 via E-QIP, please download (each form is in Adobe PDF format) and complete each of the following forms:

• Declaration for Federal Employment
• Disclosure and Authorization Pertaining to Consumer Reports
• Specialized Authority for Release of Information

Please print the forms, sign them in ink, and mail them promptly to:
