Attention Users

When entering data into the Office of the Chief Administrative Hearing Officer (OCAHO) Subpoena, Form EOIR-30, please keep in mind the following:

You can use Adobe Acrobat Reader 4.0 to enter information on the form and print it out to mail it. You can not save the information entered using this software. If you choose this option, please mail the form to the following address:

There is a charge for downloading Adobe Acrobat Exchange package (which also includes the Reader) from the Internet. Additional information can be obtained from Adobe's website.

 NOTE: OCAHO will only accept unaltered versions of this form.
