16 12/21/99 Miami Stolen from private vehicle. The weapon was placed under the front seat of the agent's vehicle while agent attended a high school football game. The agent was suspended for three days, as this was the second time the agent failed to secure a weapon adequately.
17 01/10/00 Laboratory Division Stolen from private vehicle.
18 01/10/00 Phoenix Misplaced fanny pack containing FBI-issued handgun. Special Agent received a Letter of Censure.
19 01/13/00 Cincinnati Unexplained loss.
20 01/17/00 Los Angeles Stolen from agent's residence. Weapon was in a gun bag under the agent's bed.
21 01/24/00 Miami Stolen from FBI vehicle.
22 01/24/00 Pittsburgh Unexplained theft.
23 01/24/00 Pittsburgh Stolen from FBI vehicle. The weapon was left overnight underneath numerous boxes in the vehicle. The vehicle was alarmed and parked in a parking garage.
24 01/24/00 Pittsburgh Shotgun stolen from FBI vehicle. Agent was conducting surveillance on a subject. The subject left car and entered a building. The agent could not discreetly observe subject while carrying weapon due to what the agent was wearing and the nature of the building; therefore, the shotgun was left in trunk of alarmed car. When agent returned an hour later, trunk was broken into and shotgun was missing along with weapon number 28.
25 01/25/00 Pittsburgh The Property Management Unit transferred the weapon from the firearms reference collection to the Pittsburgh inventory. There are no paper records indicating the receipt of this weapon.
26 01/25/00 Pittsburgh Stolen from FBI vehicle. Agent was observing a subject in connection to a drug investigation. When the subject left vehicle, the agent left weapon in a briefcase in the vehicle and followed the subject. When agent returned 90 minutes later, the vehicle had been broken into and the briefcase containing the weapon was missing.
27 01/25/00 Pittsburgh Stolen from FBI vehicle. The weapon was left for an entire day in the back of a Bronco. The vehicle was parked in the assigned parking area at a United States Courthouse.
28 01/25/00 Pittsburgh Pistol stolen from FBI vehicle. Agent was conducting surveillance on a subject. The subject left car and entered a building. The agent could not discreetly observe subject while wearing weapon due to what the agent was wearing and the nature of the building; therefore, the pistol was left in trunk of alarmed car. When agent returned an hour later, trunk was broken into and pistol was missing along with weapon number 24.

50 05/10/00 Washington Stolen from private vehicle. Weapon was in the center console of car. Special Agent received a Letter of Censure.
51 05/15/00 New York Weapon sent to FBI HQ. No record exists of weapon being on New York's inventory after 08/13/92.
52 05/16/00 Washington Stolen from rental car trunk. Special Agent received a Letter of Censure.
53 06/20/00 Dallas Unexplained loss.
54 06/29/00 Albany Weapon stolen from POV trunk. Special Agent received a Letter of Censure.
55 06/01/00 Honolulu Unexplained theft.
56 08/01/00 Indianapolis Agent retired in 1990. FD-500 completed 08/01/00. According to FD-500, gun stolen in 1970's.
57 08/02/00 Los Angeles Stolen from FBI vehicle. Weapon left in trunk of vehicle overnight.
58 08/03/00 Philadelphia Unexplained theft.
59 08/08/00 San Antonio Stolen from private vehicle. Weapon was left in purse inside vehicle. Special Agent received Letter of Censure.
60 09/20/00 Washington FBI vehicle containing weapon stolen from agent's residence.
61 09/28/00 San Antonio Agent retired without notice. No documentation to support weapon returned. Presumed stolen.
62 09/29/00 San Juan Agent left weapon in public phone booth. Special Agent received Letter of Censure.
63 09/29/00 San Juan Agent left weapon on fender of FBI vehicle and drove away. Special Agent received Letter of Censure.
64 09/29/00 San Juan Agent was robbed while eating lunch in a café.
65 09/29/00 San Juan Agent's residence was robbed while agent was sleeping. Received censure letter.
66 10/03/00 Los Angeles Stolen from private vehicle. Weapon left in car overnight. Special Agent received Letter of Censure.
67 10/05/00 New Orleans Stolen from agent's residence. Entry was gained into the home by forcing open a side door that contained a padlock, a locked doorknob, a locked deadbolt, and a security chain. The documentation does not reveal how the weapon was secured inside the home. FBI reported weapon found.
68 10/11/00 Baltimore Stolen from FBI vehicle parked in agent's driveway.

201 11/08/01 San Antonio Left briefcase, containing weapon, at a fast food restaurant. Presumed stolen. Special Agent received Letter of Censure.
202 11/08/01 San Antonio FBI vehicle, containing shotgun, was stolen from agent's residence. The shotgun was left overnight in the ceiling shotgun rack of the vehicle. OPR case pending.

209 12/12/01 New York Stolen from FBI vehicle during duty hours. Weapon was in trunk of vehicle. OPR determined did not warrant investigation. OPR concluded the agent took adequate steps to protect the weapon by using a chain locked trunk.
210 12/18/01 Washington Stolen from FBI vehicle parked at agent's residence. OPR case pending.
211 01/02/02 Los Angeles Stolen from agent's residence.
212 01/16/02 Jacksonville Weapon stolen out of checked luggage. The FBI was reimbursed $700 for the firearm.
