Dear Sirs,
Could you please explain why the Death Compensation Form will only be available after the "Eligibility Form
and Application for Advance Benefits" has been filed? I believe the information requested on the
Death Compensation Form could impact the decision to file the "Eligibility Form and Application for
Advance Benefits". Will you please consider making this form available to those who would like to review it first?
