It will depend on a number of factors, particularly the volume of potential victims responding to the questionnaire. If your Questionnaire is properly submitted, you should receive an initial automatic reply confirming receipt. After that, a federal case agent working with the prosecution team will follow-up with individuals if further information is required. That time frame could be weeks or months after the submission of the questionnaire.

You are not required to submit written documentation at this time. If you are contacted by a member of the investigative team, U.S. Food and Drug Administration (FDA), you may be asked to provide written documentation regarding services you received. Questions you have will be answered at that time. You will be instructed how to submit the required documentation, to whom, and by what date.

Federal court proceedings are open to the public unless otherwise noted. Court dates and times may change on short notice. The federal investigative agents and prosecutors assigned to this criminal case will make determinations after analyzing information obtained during the investigation, and in accordance with federal law and U.S. Department of Justice guidance governing the identification of crime victims. Once determinations are made, victims will receive written notification from the U.S. Attorney’s Office through the Victim Notification System (VNS).
