Clicking the Forms tab on this site's navigation bar, you can access the PDF file that contains BOTH the instructions and the compensation form.

Submissions MUST include original signatures, and the last signature MUST be notarized as indicated. You must also supply supporting documentation. Please use the checklist to help ensure you have attached the necessary documents. Please note that in most cases the documentation you submit with the claim will not be returned to you. Therefore, be sure to make copies or have additional certified copies of documents that you submit with your claim.

  • Application received
  • Claim number assigned
  • Each page electronically imaged into database
  • Varying timeframe due to complexity of claim submissions





  • Fund staff review claim for eligibility requirements
  • Identifies missing items and requests additional information as necessary







  • Reviews claim
  • Makes eligibility decision
  • If eligible, claimant is notified of eligibility and missing documents are requested.
  • When necessary documents are submitted, claim is deemed substantially complete.
  • If Track A, presumed award letter is issued. Claimant accepts or requests a hearing.
  • If Track B, claimant will be notified to schedule a hearing date
  • Finalizes Award


  • Letter containing VCF assigned claim number




  • Initial contact from Case Manager outlining missing items
  • Follow-up correspondence as necessary
  • Substantially Complete letter
  • Presumed Award letter
