Form Indictment—Solicitation or Receipt of Prohibited Payment by Labor Union Official in Private Sector (Taft-Hartley) (§  186(b)(1) and (d)(2) based on § 186(a)(2))—29 U.S.C. § 186


That on or about (date) in the _____________________ District of __________ the defendant __________________________, an [officer/employee] of a labor organization, to wit: (name of union) , did unlawfully, willfully, and knowingly request, demand, receive and accept, and agree to receive and accept the payment, loan, and delivery of a thing of value [and money in the approximate amount of $_________] from (name of payor) ,

[an employer] [an association of employers] [a person acting as a labor relations expert, advisor and consultant to an employer]

[a person acting in the interest of an employer] whose employees were employed in an industry affecting commerce * and whom such

(name of union) [represented/sought to represent/would have admitted to membership] the employees of (name of employer) .

All in violation of Title 29, United States Code, Sections 186(b)(1) and (d)(2). **
