Equitable Sharing Agreement and Certification Form (OMB 1123-0011 March 2009)
All State and local law enforcement agencies that receive equitable sharing must submit this report within 60 days after the close of their fiscal year to both the Department of Justice and the Department of Treasury. This report itemizes annual receipts and expenditures of Federal shared funds by State and local law enforcement agencies. It is also the equitable sharing agreement for the agency's next fiscal year. This form replaces the previous Annual Certification Report and the triennial Federal Equitable Sharing Agreement.
Application for Transfer of Federally Forfeited Property Form DAG-71
State or local law enforcement agencies that directly participate in an investigation or prosecution that results in a federal forfeiture may use this form to request an equitable share of the net proceeds of the forfeiture. A separate application must be submitted for each asset within 60 days of the seizure. A different application must be filed with the Department of Treasury for assets seized by Treasury agencies. See Form TDF 92-22.46.
U.S. Marshals Service ACH Vendor Form
This U.S. Marshals Service form enables a participating law enforcement agency to establish electronic fund transfer capability. This form is required in order for equitable sharing payments to be disbursed to participating state and local law enforcement agencies. This form can also be used to update existing banking information. Once completed, this form must be faxed to the U.S. Marshals Service at 202-307-8517.
