OBJECTION FORM
You have the right to submit a written objection to your individual award determination. If you wish to object, you must submit your objection in writing no later than October 19, 2021.

- Check your mail and/or email. The United States will send letters to all claimants who completed an Interest-in-Relief Form notifying them of their individual award determinations and providing Notice of Fairness Hearing on Individual Awards, which will include the Objection Form.
Instructions for Completing the Objection Form
- Fill out completely every section of the Objection Form, including a description of the reason(s) for your objection.
- Submit the the Objection Form to the United States by October 19, 2021:

- The court will hold the Fairness Hearing on Individual Awards by Zoom videoconference technology on December 3, 2021, at 1:00 pm. You may attend this hearing and speak if you wish, but you do not need to attend to have the court consider any written objections that you choose to submit.
