PLEASE NOTE: Although the volume of information we receive from concerned members of the public prevents us from responding individually to every Report, be assured that we will carefully consider the information you have provided to determine whether there is a matter for Federal investigation. Should we determine that your Report raises a matter within the jurisdiction of this Office and that further information from you is necessary, you will be contacted by a Federal Law Enforcement Agent. This Office does not resolve individual consumer complaints.

Unless reporting a crime anonymously, after you print out the form, please make sure to sign at the bottom before sending by regular mail to:
